Common use
Domperidone is used to relieve symptoms of nausea, vomiting, bloating, and discomfort caused by delayed gastric emptying or other gastrointestinal conditions. It works by blocking dopamine receptors in the gut and chemoreceptor trigger zone, which enhances stomach motility and reduces nausea. The drug is also used to reduce gastrointestinal side effects in patients receiving Parkinson’s disease treatment. Domperidone is typically prescribed short-term and under medical supervision.
Dosage and direction
The typical adult dose is 10 mg taken up to three times daily, 15–30 minutes before meals. The total daily dose should not exceed 30 mg unless prescribed by a doctor. Tablets should be swallowed whole with water, not chewed. The duration of treatment is generally limited to a few days up to one week.
Precautions
Before taking domperidone, inform your doctor if you have liver disease, heart rhythm disorders, or gastrointestinal bleeding. This medication may increase the risk of QT prolongation and serious cardiac arrhythmias, especially in older adults or when used with certain other medications. Domperidone should be avoided during pregnancy unless clearly needed. Caution is advised while breastfeeding, as small amounts may pass into breast milk.
Contraindications
Domperidone is contraindicated in patients with known hypersensitivity to the drug, gastrointestinal obstruction, perforation, or bleeding. It should not be used in patients with moderate to severe liver impairment. The drug is not recommended in combination with medications that prolong the QT interval or inhibit CYP3A4 enzymes (e.g., ketoconazole, erythromycin). Domperidone should also be avoided in patients with existing cardiac conditions such as long QT syndrome.
Possible side effect
Common side effects include dry mouth, abdominal cramps, dizziness, and headache. Less common effects include breast tenderness, menstrual irregularities, and galactorrhea due to elevated prolactin levels. Rare but serious side effects include changes in heart rhythm and allergic reactions. Seek immediate medical attention if you experience fainting, palpitations, or difficulty breathing.
Drug interaction
Domperidone interacts with drugs that inhibit CYP3A4, such as antifungals (ketoconazole), macrolide antibiotics (erythromycin), and some HIV protease inhibitors. It should not be used with other medications known to prolong the QT interval. Alcohol may increase the risk of drowsiness and cardiac side effects. Always inform your healthcare provider about all medications and supplements you're taking.
Missed dose
If you miss a dose, take it when you remember unless it’s close to your next dose. Do not double the dose to compensate. Continue with the next scheduled dose as usual. Regular dosing improves effectiveness.
Overdose
Overdose symptoms may include drowsiness, confusion, and abnormal body movements, especially in children. In severe cases, irregular heartbeat or cardiac arrest may occur. Immediate medical help is required. Treatment includes symptomatic care and cardiac monitoring.
Storage
Store domperidone at room temperature, away from heat and moisture. Keep it in its original container and out of reach of children. Do not use after the expiration date. Avoid storing in the bathroom.
